Mount Holyoke Student-Athlete Questionnaire

Congratulations on your selection to a Mount Holyoke athletic team! We are interested in publicizing your accomplishments, and we regularly send news releases to hometown newspapers, as well as local and regional media outlets. Some information that you provide may be used on the College's website. The following questions will help provide necessary background information, which will be used to help publicize you and your team.

Again, this form helps the Athletic Communications Office with your bio online, as well as your background when building rosters. The information is also used for awards, such as Academic All-America; Academic All-District, as well as any Scholar Athlete awards that your sport may have.
