The following instructions must be followed by all those interested in trying out for a varsity intercollegiate team at Mount Holyoke College. Forms must be submitted prior to the traditional or non-tradtional season, whichever comes first in the academic year.
NOTE: This medical clearance process does not apply to club sport athletes.
First Year/ New Athletes
- Pre-entrance Health Requirement: All immunization records and paperwork can be found on myHealthConnection through MHC health services (https://www.mtholyoke.edu/health/incoming-health-forms). You can contact health services at health@mtholyoke.edu or fax: 413-538-2352
- All new athletes need to complete a concussion baseline test.
- Complete the concussion baseline test
- Forms include:
- Immunizations (hard copies from physician and electronically inputted by student)
- Medical History Form
- Sports Physical (signed by physician)
- Sickle Cell Waiver Form (or a copy of sickle cell test results from physician)
Needed Yearly from All Athletes:

Returners Need Yearly:
- Medical Exception Documentation: Banned substances, add/adhd
- Annual Blood Pressure check (done in ATR in april)
- Update EIS if any information changes
